IMPORTANT NOTICE TO ALL 2012-2013 ATHLETES  

Before participating in sports, a Pre-Participation form must be completed. Each school year a Health Appraisal of every athlete is required to participate in school athletics. Physicals can be provided by the school, however, a complete physical with your child's physician is recommended. Each season, an Interval Health History must be completed as well.
